The Wolverine (review)

Honestly, due to financial issue I've decided to reduce my budget to go to cinema. That's why I don't have plan to watch The Heat, The Wolverine or The Conjuring. But then I read in Flipboard that there's a sneak peak of X-Men: Days of Future Past in the end credit of The Wolverine. Call me crazy. As soon as I read that, I immediately check the theater schedule, take a bath, then rushed to go to cinema.


I think The Wolverine is quite good but somehow I feel it's a let down. It's more a drama than action and there's a twist in the story. Most people will be surprised but I'm not. You know that you've watched too many movies when you can guess the twist in the middle of the story (right after Yukio said she can't foretell the death of one character). The music is forgettable. The action is quite good. Personally, I don't like the fight between Wolverine and the yakuza on top of the Shinkansen. It's a little bit unrealistic. But I love to see Yukio fight. Is it just me? I think I prefer Wolverine-Yukio pairing than Wolverine-Mariko. He he he. Oh, and the Viper is quite a sexy villain.

Overall, I think this movie is a so-so movie. It didn't meet my high expectation. Much better than X-Men Origins: Wolverine, but I love X-Men First Class a lot more. And as for the ending credit scene? For me, just seeing Sir Ian McKellen come back as Magneto and Patrick Steward as Professor X is a treat. In X-Men: Days of Future Past, Magneto and Professor X is comrade rather than enemy. I want to know how Bryan Singer will play that out. So, have you seen The Wolverine? What do you think about it?

Risiko Efek Samping Thromboembolism dan Penggunaan Cyproterone Acetate + Ethinylestradiol

Sekitar pertengahan bulan Juni kemarin aku membaca suatu artikel tentang seorang remaja yang meninggal karena mengalami pulmonary embolism dan deep vein thrombosis yang kemungkinan akibat meminum obat berisi cyproterone acetate dan ethinylestradiol yang dia gunakan sebagai anti jerawat. Setelah aku membaca lagi berbagai artikel dan jurnal, ternyata kematian yang diasosiasikan dengan pemakaian obat ini memang cukup sering masuk surat kabar. Obat ini banyak digunakan oleh wanita terutama yang berusia cukup muda karena selain dapat digunakan sebagai anti jerawat juga dapat berfungsi sebagai kontrasepsi. Dapat mencegah kehamilan sekaligus membuat kulit bebas jerawat? Tak heran jika obat ini terkenal, banyak diresepkan dan banyak dicari.

Pada tanggal 30 Januari 2013, Badan Otoritas Perancis, Agence Nationale de Securite du Medicament et des produits de Sante (ANSM) menerbitkan press release tentang pembekuan izin edar produk obat yang mengandung kombinasi Cyproterone Acetate 2 mg + Ethinylestradiol 35 mcg. Tindakan ini diambil karena database Farmakovigilans Perancis menunjukkan terdapat 125 laporan thromboembolism yang terdiri dari 113 laporan venous thromboembolic events (VTE) dan 12 laporan arterial thromboembolic events (ATE). Analisis literatur menunjukkan bahwa terdapat peningkatan risiko venous thromboembolism empat kali lebih tinggi pada wanita yang menggunakan produk obat tersebut, dibandingkan dengan wanita yang tidak menggunakan.

Sebenarnya, semua obat kontrasepsi memiliki risiko menyebabkan deep vein thrombosis. Tetapi dari kajian European Medicine Agency (EMEA) disebutkan bahwa studi observasional menunjukkan risiko VTE pada produk obat tersebut adalah 1,5 - 2 kali lipat lebih tinggi dibandingkan dengan kontrasepsi oral kombinasi yang mengandung levonorgestrel meskipun risiko VTE-nya mirip dengan risiko kontrasepsi yang mengandung gestodene, desogestrel, atau drospirenone. Sehubungan dengan hasil kajian tersebut, kepada profesional kesehatan direkomendasikan hal-hal berikut:

  • Produk obat ini hanya digunakan untuk pengobatan jerawat sedang hingga berat terkait dengan sensitivitas androgen dan atau hirsutisme pada wanita usia subur. Pengobatan jerawat dengan obat ini hanya digunakan ketika terapi topikal dan pengobatan antibiotik oral telah gagal.
  • Karena obat ini juga merupakan kontrasepsi hormonal, maka tidak boleh digunakan secara bersamaan dengan kombinasi kontrasepsi hormonal lain. Hal ini akan menyebabkan wanita terpapar estrogen dengan dosis yang lebih tinggi.
  • Dokter sebaiknya melakukan kajian pada pasien yang menggunakan produk obat ini sesuai dengan rekomendasi di atas, pada jadwal kontrol berikutnya.
  • Dokter sebaiknya berdiskusi dengan pasiennya terkait risiko thromboembolism dan faktor risiko seperti peningkatan usia, merokok, obesitas dan prolonged immobility.

Badan otoritas negara lain seperti Kanada dan Australia juga telah melakukan kajian terkait risiko efek samping thromboembolism pada produk obat yang mengandung kombinasi Cyproterone acetate + Ethinylestradiol dan menyimpulkan bahwa manfaat obat tersebut masih lebih besar daripada risikonya jika digunakan sesuai dengan yang dianjurkan. Di Indonesia, Badan POM masih melakukan kajian untuk menentukan langkah regulatori yang tepat. Perlu diketahui bahwa terkadang kebijakan yang diambil suatu negara bisa berbeda dengan negara lain. Ada obat yang dilarang di suatu negara tetapi masih dipergunakan di negara lain. Hal ini karena perbedaan karakteristik populasi antara satu negara dengan negara lain sehingga intensitas dan frekuensi kejadian efek samping yang terjadi juga berbeda. 

Sambil menunggu kebijakan tentang obat ini di Indonesia, ada baiknya profesional kesehatan (baik dokter, apoteker, maupun bidan, dll) selalu menginformasikan risiko efek samping obat yang berbahaya, mengedukasi pasien terkait gejalanya dan apa yang harus dilakukan pasien jika gejala tersebut muncul. Jangan lupa laporkan efek samping produk obat yang terjadi di lapangan ke ke Badan POM RI. Mendeteksi Kebohongan dalam Iklan dan Klaim Kosmetika

Belakangan ini banyak sekali iklan kosmetika yang mempromosikan seolah-olah produknya adalah obat. Dengan berbagai klaim bombastis dan janji-janji yang terlalu indah untuk jadi kenyataan, iklan tersebut menyesatkan konsumen agar berbondong-bondong membeli produknya . *geleng-geleng kepala*. Sungguh prihatin. Sebenarnya sah-sah saja jika produsen atau distributor membuat suatu klaim tentang manfaat produknya, namanya juga orang jualan. Tapi sangatlah tidak bijaksana jika manfaat yang diiklankan tersebut jauh dari kenyataannya. Itu namanya pembohongan publik!

Sebelum membahas soal kebohongan-kebohongan di atas, ada baiknya kita berkenalan dulu dengan yang namanya klaim. Menurut Peraturan Kepala Badan POM Tahun 2010 tentang Persyaratan Teknis Kosmetika, klaim kosmetika adalah pernyataan pada penandaan berupa informasi mengenai manfaat, keamanan dan/atau pernyataan lain. Klaim ini dapat dibagi menjadi 2, yaitu:

  • Klaim Kemanfaatan
Contoh klaim kemanfaatan antara lain: membantu mencerahkan kulit, mewangikan badan, melembutkan rambut, dll. Intinya informasi tentang manfaat yang bisa didapatkan konsumen jika memakai kosmetika tersebut.
  • Klaim Keamanan
Contoh klaim keamanan antara lain: tidak pedih di mata (biasanya pada produk sampo bayi), hypoallergenic (kurang menimbulkan alergi), dll. Intinya informasi tentang keamanan kosmetika yang dapat menarik konsumen agar lebih memilih produk tersebut dibandingkan kompetitornya. Ada beberapa klaim yang dimaksudkan produsen sebagai klaim keamanan, tetapi salah kaprah dan menyesatkan konsumen. Contohnya adalah klaim "bebas merkuri". Hal ini sebenarnya tidak dapat digunakan sebagai klaim keamanan karena semua kosmetika memang tidak boleh mengandung merkuri. Jadi klaim ini menyesatkan, seolah-olah kosmetika tersebut lebih aman padahal sebenarnya tidak. Demikian pula dengan klaim "tidak mengandung pengawet" atau "tidak mengandung paraben". Pengawet dan paraben merupakan bahan yang diperbolehkan dalam kosmetika. Tidak ada perbedaan keamanan yang bermakna antara kosmetika yang mengandung ataupun tidak mengandung pengawet. 

Oke. Setelah berkenalan dengan jenis-jenis klaim di atas, mari kita membedakan klaim suatu produk. Apakah dia mengklaim sebagai kosmetika atau mengklaim suatu obat? Ada 5 kriteria yang digunakan regulator untuk membedakan apakah produk tersebut termasuk kosmetika dan apakah klaimnya termasuk klaim kosmetika seperti bagan di bawah ini:




Bagian yang harus diperhatikan ketika kita menilai klaim suatu kosmetika adalah bagian 3, 4 dan 5 di atas. Jika klaim yang dipromosikan melalui iklan di luar ketiga hal tersebut (di luar fungsi kosmetika, peruntukan untuk mengobati atau mencegah penyakit, serta klaim dapat mengubah fungsi fisiologi tubuh), maka bisa dipastikan klaim dan iklan itu bohong.

Berikut adalah suatu contoh iklan kosmetika di facebook yang menurutku sangat tidak etis, tidak masuk akal, bohong dan menyesatkan:




Dari contoh kasus di atas, ada beberapa hal yang harus dibahas:

  • NA itu artinya bukan notifikasi Asia, itu hanya kode dari Badan POM yang artinya produk tersebut terdaftar di Indonesia. Belum ada kejadian suatu produk yang terdaftar di suatu negara berarti otomatis terdaftar juga di negara atau kawasan lain (kecuali eropa karena adanya Europe Union).
  • Dari ke-12 klaim yang dicantumkan diatas, hanya beberapa yang merupakan klaim kosmetika. Sisanya merupakan klaim pengobatan atau mengubah fungsi fisiologi tubuh manusia yang tidak dapat dilakukan oleh kosmetika.
  • Produk di atas merupakan kosmetika yang terdaftar sebagai feminine hygiene, alias kosmetika untuk menjaga kebersihan daerah kewanitaan. Adalah suatu kebohongan jika sediaan ini dapat mengubah fungsi fisiologi (seperti melancarkan sekresi vagina), membunuh bakteri patogen, apalagi mencegah dan mengatasi kista. Hanya produk yang terdaftar sebagai obat yang boleh menggunakan klaim-klaim tersebut.
Salah satu perbedaan besar antara obat dan kosmetika adalah klaim obat harus dibuktikan dengan berbagai macam penelitian dan uji klinis sementara proses pembuktian klaim pada kosmetika relatif lebih mudah. Jadi jika kalian melihat suatu kosmetika diiklankan memiliki manfaat seperti obat, dapat dipastikan bahwa iklan tersebut pasti bohong. Berhati-hatilah dalam membeli produk dan jadilah konsumen yang pintar.

Honestly, I have a mixed feeling about this song. At first I thought this song is catchy and easy listening, but I become a little bit bore after hear it for 5 days :-(. If you're curious, you could download the song here and the video here.

Avenged Sevenfold - Hail to the King

Hail to the King is the upcoming 6th studio album by one of my favorite band, Avenged Sevenfold, and also the name of the first single.


I think they take a different direction again with this song. But they haven't release the entire album so I can't make comment about the changes in their music. Overall I think this is a good song. My only complaint is that the guitar solo sounds a little bit lame considering Synyster Gates had made many better guitar solo than this. This is also the first time I heard Arin Ilejay's drum play even though he already become Avenged Sevenfold's touring drummer and has recorded 2 songs with them.

Pasific Rim (Review)

When I read today that more people choose to watch Grown Ups 2 than Pasific Rim, I thought maybe the Earth is really coming to an end. Seriously, after a series of mediocre to bad films that neither good nor funny, I've lost faith to Adam Sandler's ability to make me laugh. On the other side, Pasific Rim had many cliche moments but it still really fun that I don't really mind and still think this movie will make it to my Top 10 Best 2013 Movie list. To be fair, my opinion maybe have something to do with the fact that I'm a fan of anime. Maybe that's why more people watch Grown Ups 2, because it attracts attention from everyone but Pasific Rim only get attention from nerds, anime fans, and anyone who wants to watch giant robots fighting giant monsters. Whatever, just read my review about this film below!


Guillermo del Toro is a very visual director so it's not surprising that the visual in this movie is good. Some people complain about the darker tone he use in this movie. I really don't mind because a lot of the fighting scenes were in night time. If it's too bright it would not feel realistic. The action sequences is good. Unlike Transformers movie where the action is messy and over the top (so sometimes I don't get what's happening), the action scenes in this movie is not hard to follow. It's not boring and repetitive like what I felt when watched Man of Steel. The music score is good. Some would feel that the score of this film is too loud and noisy but I was so immersed in the action scenes that I don't mind at all.

This movie is imaginative but some parts of it is believable. I can really picture what would human do if someday Kaiju appear in this world, it might be not so deviated from this movie. The characters are interesting though there's many cheesy dialogues in this movie. I was impressed by the performance of Mana Ashida, the young Japanese artist who played the young Mako. Idris Elba is very good too.

What I don't like about this movie is the ending. Too convenience and made me raise a lot of questions because of the implausibility. But overall this movie is fun and enjoyable. Have you watch Pasific Rim? What do you think about it? Comment bellow. Let me know.

Two Door Cinema Club

Two Door Cinema Club is one of my favorite indie band from Northern Ireland that consist of Alex Timbre (Lead vocalist, guitarist, beats synths), Sam Halliday (Lead guitarist, vocalist) and Kevin Baird (Bassist, vocalist, synths). Until now, they have released 2 albums, Tourist History (which receive an award) and Beacon.

Pixies - Where is My Mind

I plan to write a series of posts about some of my favorite indie songs and indie bands. To start this series, I decide to write one of my favorite song from Pixies, Where is My Mind. Honestly I don't really know about them before. Of course I've been hearing about them as one of the most influential alternative rock band. But I never come across their music until I watch Sucker Punch. One of the soundtrack of this movie is a cover of Where is My Mind and it's pretty good that I want to hear the original song. You could download the cover song by Yoav and Emily browning here. The style of this cover is more dreamy pop than alternative rock like the original song. But it's still really good.

Perubahan pada Label Codeine Sulfate

Di Amerika, dilaporkan terdapat kematian pada anak-anak yang berhubungan dengan Ultra-Rapid Metabolism of Codeine to Morphine respiratory depression. Peristiwa ini terjadi pada anak yang mendapat Codeine setelah tonsilektomi atau adenoidektomi dan memiliki polimorfi enzim CYP2D6. Polimorfi enzim ini menyebabkan metabolisme Codeine menjadi Morphine berlangsung sangat cepat sehingga berpotensi meningkatkan kadar Morphine pada darah dan menyebabkan kematian karena depresi pernapasan. Karena itu, FDA memutuskan Codeine Sulfate dikontraindikasikan untuk manajemen nyeri paska operasi pada anak-anak yang menjalani tonsilektomi atau adenoidektomi. Sejawat tenaga kesehatan agar berhati-hati dalam meresepkan Codeine Sulfate bagi anak-anak dengan selalu menimbang risiko dan manfaatnya.

Rekomendasi penggunaan Olanzapine Pamoate (Zyprexa Relprevv)

Sejak bulan lalu, FDA menginvestigasi 2 kasus kematian yang tidak dapat dijelaskan pada pasien yang mendapat injeksi i.m Zyprexa Relprevv (Olanzapine Pamoate). Pasien meninggal 3 - 4 hari setelah mendapat injeksi pada rentang dosis yang sesuai setelah dimonitor pada 3 jam post injeksi sesuai dengan Relprevv Risk Evaluation and Mitigation Strategy (REMS). Kadar Olanzapine yang sangat tinggi ditemukan pada darah kedua pasien yang meninggal. Label dari obat ini sendiri mencantumkan peringatan tentang risiko Post-injection Delirium Sedation Syndrome (PDSS) dimana obat dapat terlalu cepat memasuki tubuh pada injeksi intra muskular yang menyebabkan tingginya kadar obat dalam darah.

Olanzapine pamoate yang diinjeksikan secara i.m adalah antipsikotik dengan aksi panjang yang formulasinya didesain untuk melepaskan obat secara perlahan-lahan sehingga memiliki efek selama 2 - 4 minggu. Terdapat risiko peningkatan kadar obat dalam darah yang tinggi jika pelepasan obat yang terjadi di pasien berlangsung terlalu cepat. Karena efek samping serius yang dapat muncul, hanya fasilitas kesehatan dan pasien khusus yang dapat menerima obat ini. Sambil menunggu hasil investigasi, FDA merekomendasikan agar pasien yang menerima obat ini dimonitor sesuai dengan persyaratan REMS dan label obat.

Despicable Me 2 (Review)

After Monster University, I had a high hopes for Despicable Me 2. Is the reality match my expectation? Keep read this post to find out!

Actually this movie is good enough. The minions is the highlight of this movie and they're so funny. Gru's 3 children are so adorable (particularly Agnes). The villain is a badass, I particularly like the introduction of El Macho. The gadgets are cools even thought there's only few of them. I don't really like the music score though. The music and the story in this film is pretty lame, the solution of the problem is too convenient. Children will love it but adults will feel that they don't have many things to dig from this movie. The 3D is useless so don't watch it in 3D because it won't add more good viewing experience. Finally I would say that I'm a little bit disappointed. But the funny minions in this movie made it still worth watching.

Monster University (Review)

Finally! The first descent animation movie in 2013 is none other than Monster University!

Monster University is the prequel of Monster Inc that told story from the days Mike Wazowski and James Sullivan train as scarer. It's kind of odd that in this movie they first meet each other in university since I remember Mike said to James in Monster Inc movie that he always jealous of Mike's face since they're in their 4th grade which emphasize that they've known each other since they were kids. But whatever, It's still a good movie both for kids and adults.

It opens by a short animation movie, "the Blue Umbrella". It's quite a good and unique movie. I personally love the intro of the movie that tell us how Mike Wazowski became fascinated by the scarer  and dream to take the scare major in Monster University. After that, the movie told us a generic story of how losers and underrated group become the winner of the contest. It's far from bad story, but generic. I must point out that the story of Monster Inc is more unique and fresh. But the last 30 minutes of this movie is golden, deep, touching and entertaining.

Overall, I'm satisfied with Monster University. I only wish they could bring a more fresh story but this movie is executed in a descent way. Watch it! It's worth your time.

World War Z (Review)

World War Z is a movie based on novel with the same name. However, the story in the movie is deviated so much from its original noveI, lead to outcries from the fans of the book. I never read the book so I can't give comment about that but the movie is enjoyable enough. Maybe I would say otherwise after read the book though, like when I saw Percy Jackson and the Lightning Thief movie that I thought was fine at that time. After read the books in the series, I became furious because they almost ruin the original story. But in the meantime, let me told you my impression of World War Z as a movie.


Brad Pitt act as the main character, a United Nations agent that have to investigate the source, cause and cure of the zombie apocalypse. This movie is cool enough for a bloodless zombie movie. If majority of books and movies portray zombies as slow creature, in this movie they fast and act more aggressive to sound. The premise is cool and the story touch a little bit about politic here. Just imagine what measures the government will take to overcome this whole zombie outbreak. This movie is meant to be touching but I found that the addition of the family in this story did not lead to a touching drama. It's all about building the tension and suspense. The cinematography is good. The music score is so so. It's too bad that this movie don't give room for minor characters to shine. The ending is not satisfactory, but maybe it's because they want to make the sequel of this film. Overall, watching World War Z is a good time for me.

Man of Steel (Review)

Just as Iron Man 3, half the viewers love it while the other half hate it. I was hesitate to watch it at first but finally I made up my mind and watch it 1 week ago.


Overall, for me it is a good movie. Even thought I could not grasp all those things about Codex and basically all the things about Krypton. The movie spend a good amount of time to introduce Krypton to us and it looks impressive. I don't know whether it's because I'm dumb or the story is not plausible, I really don't get why Jol-Er put the Codex inside Kal-Er then send him to Earth. Of course I know that he want to save his child before the destruction of Krypton, but he intend to save the Krypton legacy by incorporating the Codex, thing that store the DNA code of Kryptonian, to Kal-Er. How he plans to bring back the whole Kryptonian race by doing so is still a mystery for me. Other things that I still don't understand is General Zod and his baseless hatred to Kal-Er. I found it's weird that Kryptonian had time to adjudicate and sentence General Zod and his follower while they supposed to evacuate themselves since Krypton planet is on its edge of destruction (and foolishly made them an extinct race).  

Apart from the Krypton thing, I found Man of Steel is enjoyable. The acting is good but too bad there's not much room for romantic story between Clark Kent and Louis Lane. The music and the cinematography is so-so. The action is cool but I found it tiring. Maybe because I fed up with mega CGI-action scenes such as in Transformers, The Avengers, practically almost all Hollywood movie. Seriously, why the destruction is always focused in America?? (Not that I complain though. At least in the movies those aliens never come to my home country).

It's not groundbreaking or a must-see film. That's being said, I will still recommend you to watch it and see it for yourself.
